The Community Support Agreement (commonly referred to as CSA by Sales Reps) is the billing form used for Free Product/Services Placement WITH lock-in period.


This form is used for:

  • Free Equipment Placement with a lock-in period.
  • Free Install/Deployment Fee with a lock-in period.
  • Free Marketing Products/Services with a lock-in period.


Again, be reminded that this is WITH lock-in period which requires the merchant to process with us in a certain period of time. If you are looking for free placement with NO lock-in period, this is not the form you would want to fill out.


Below is how you are to fill out this form:


GENERAL INFORMATION:

Business Legal Name Referred to as the “IRS Name”. This is the name they file their taxes under, which can be completely different from the DBA.
Hardware
Value
List of equipment you are providing for free.
Sum of the equipment costs.
Installation
Value
This is usually technical fees, such as but not limited to, deployment fees, site visit fees, and out-of-state install fees.
Sum of the technical fees.
Other
Value
List of products/services not classified as hardware, installation, and Payzero. This is usually menu/flyers for non-PayZero clients and previous processor fees and/or ETF.
Sum of the products/services listed.
PayZero Services
Value
By default, this is the Marketing menu/flyers.
For new merchants, this is default to $300.00. If value is indicated, specify accordingly.
Total Value Sum of ALL products/services provided for free.
Calendar Months The number of months will depend on the calculated Equipment ROI. Make sure to calculate Equipment ROI first on all incoming accounts.
Processing Rate For new account, check off "rate set forth in the MPA".
For existing accounts, indicate the processing rate.


ACCEPTANCE AGREEMENT:

Owner's Signature Owner/Authorized Signer's signature.
Print Name Owner/Authorized Signer's full name.
Date Signed Date the document is signed.


PERSONAL GUARANTY:

Personal Guarantor's Signature Not necessarily the owner, but defaulted to be the owner/authorized signer.
Print Name Not necessarily the owner, but defaulted to be the owner/authorized signer.
